The first of its kind in our community, the RiverStone Health Hospice Home opened in 2011. This new Home, accredited by the prestigious Community Health Accreditation Program (CHAP),provides hospice care in a home-like setting with a focus on comfort, compassion, and dignity that meets the highest standard of excellence. Our philosophy of care enables patients and their loved ones to celebrate life.

People turn to hospice care when home care, hospitals, and nursing homes are not the right options or personal choice for end-of-life care. RiverStone Health Hospice Home is designed to provide the intimacy desired for private and cherished family time.
Our community is a special place. Our history, culture, and natural beauty that surround us, all contribute to a strong sense of individual spirit. We live independently and with an inpatient hospice home we can spend the last days of our life on our terms; with family and friends in a warm, loving environment.

 

Join Our Commitment

The future of RiverStone Health Hospice Services includes having the only inpatient Hospice Home facility located in a two hundred mile radius. Funding has come from a variety of sources, including RiverStone Health, grants from private foundations and local business, and community contributors such as you.

We have paved the way into our Home with bricks. You can help by purchasing a brick paver along the path of the Serenity Garden. Engraving a personalized brick is a meaningful way to ensure that your legacy or the legacy of a loved one lives on.
